Five measures, named
Tied to your last survey, your discipline mix, and the surveyor focus areas active in your region. Each measure has a numerator, a denominator, a data source, and a named owner.
The committee cadence
Monthly QAPI committee agenda template, quarterly governing-body report template, and the rolling twelve-month calendar that keeps the program moving without anyone chasing it.
The intervention library
For every measure, a documented intervention you can run when the trend slips. With the evidence trail surveyors expect to see when an intervention closes a finding.
How it runs
Six weeks from kickoff to installed program.
Weeks 1–2 · Diagnose
We review your last survey, your existing QAPI artifacts, and a sample of recent committee minutes. Output: a one-page diagnosis of what’s working and what’s a citation risk.
Weeks 3–4 · Design
Five-measure design session with your clinical leadership. We pick the measures together, draft the data-collection plan, and lock the committee cadence.
Weeks 5–6 · Install
Templates loaded into your EHR or document system. First QAPI committee meeting facilitated by Irene. First governing-body report drafted with your team.
QAPI proves improvement. It does not report activity. The gap between a passing QAPI program and a cited one is almost always the form of the evidence.

Does this replace our existing QAPI program?
Usually it absorbs the parts that work and replaces the parts that don’t. We start with a one-page diagnosis so the design session is informed, not theoretical.
Who owns the program after the engagement?
You do. Every template, every measure, every intervention is documented inside your EHR or document system, with named owners on your team. ILS CARE doesn’t host or gate anything.
Will this work for both hospice and home health?
Yes. CoP requirements differ but the structural design (measures, cadence, interventions, evidence) is the same shape. Measure selection is tuned to discipline.
Do you offer ongoing committee support after the install?
Yes, that is the External QAPI Program. Most agencies use the design engagement first, then move into ongoing committee support after the program is running.
