Tier 2 · Flagship engagement
Eight weeks to survey-ready: documented, defensible, repeatable.
A structured engagement built around the D.O.C.U.M.E.N.T framework. Chart audit sprint, CTI rewrites, IDT cadence, QAPI program design, corrective action plans delivered directly by Irene and packaged so your team can run the system after we leave.
of Readiness Program clients pass their next full survey with zero condition-level deficiencies.
of Medicare revenue protected across Readiness Program clients since 2019.
average time from kick-off to a fully documented, defensible QAPI program in your EHR.
The D.O.C.U.M.E.N.T framework, operationalized.
Nine deliverables mapped to the eight dimensions of the framework. Every piece of documentation comes back to your team ready to use the Monday after we leave.
Diagnosis-specific decline sheets
One-page reference cards for the ten most-cited terminal conditions. Each one turns a CTI into a defensible record instead of a narrative guess.
Ongoing chart audit cadence
A 30-day rolling audit protocol your QAPI lead owns after we leave. It is a daily standard, not an annual scramble.
CTI narrative templates
Eleven certificate-of-terminal-illness frames, disease-specific, pre-validated against MAC reviewer patterns.
Unified IDT meeting template
Meeting agenda, documentation pattern, and a post-meeting checklist that matches surveyor expectations exactly.
Measurement and QAPI design
A full QAPI program template with the five measures surveyors actually check and the evidence path for each.
Emergency preparedness records
Competency checklists, drill documentation templates, and the record format that survives a CHAP review.
Narrative coaching for clinicians
Two live sessions plus async review. Teaches your nurses to write notes that hold up under review, not just notes that document the visit.
Training package with CEUs
Four CEU-bearing modules delivered to your LMS. Staff can re-take after onboarding; credits count.
90-day post-engagement support
Ninety days of async email access to Irene for question-level help on anything that comes out of your next audit cycle.
Eight weeks, four phases, one working system at the end.
A predictable cadence. No open-ended discovery. You know what’s happening each week and who owns what.
Audit and triage
Full chart audit across a representative sample. We surface the top fifteen risks sorted by likelihood of citation and revenue exposure. You get a written triage report with a prioritized fix list.
- Chart audit
- CTI review
- IDT record review
- Written triage report
Template build and install
We rebuild your CTI narrative patterns, IDT meeting template, and QAPI program documents. Everything goes into your EHR and your clinical leadership binder. Your team co-authors so adoption is real on day one.
- CTI templates
- IDT template
- QAPI design
- EHR install
Training and narrative coaching
Two live sessions with your clinical team plus async review of a sample of new notes. Purpose is behavior change, not information transfer. We close the loop until the next visit note looks different.
- Two live sessions
- Async note review
- CEU modules
Handoff and mock survey
Full mock survey against the new system. Gap list, remediation plan, and the first month of audit cadence scheduled. You leave with a working system and a named owner for each piece.

- Gap remediation
- Audit cadence set
- 90-day support begins
We came out of CHAP with zero condition-level deficiencies for the first time in four cycles. The templates were the obvious win; the unspoken one was that our DON finally owns the audit cadence. It’s not Irene’s system. It’s ours now.
Traditional compliance consulting vs the Readiness Program.
| Dimension | Traditional consulting | ILS CARE |
|---|---|---|
| Who delivers | A junior associate you don’t meet until kick-off. | Irene, every session. No associate chain. |
| What you walk away with | A 60-page findings deck. Implementation is “on you.” | Installed templates, trained staff, audit cadence in motion. |
| Post-engagement | The clock stops the day the invoice clears. | 90 days of async access included. |
| Methodology | Generic CMS checklist walked in binder form. | The D.O.C.U.M.E.N.T framework, disease-specific, surveyor-pattern-aware. |
| Price transparency | “Let’s talk.” Budget sliders on intake. | $18,500 flat. Published on the page you’re reading. |
What teams ask before signing.
Is eight weeks actually enough?
Yes, because the program isn’t “learn everything.” It’s “install nine specific pieces of documentation infrastructure and train your clinical team on the two or three habits that drive most citations.” Eight weeks is the empirically right dose; shorter leaves adoption gaps, longer runs into diminishing returns.
What if my team doesn’t have time for live sessions?
The live sessions are two, ninety minutes each, scheduled around your IDT meeting cadence. We record and package them into CEU modules your team can retake async. The async note review doesn’t require any additional time from your clinicians. We review notes already being written.
Does this work for multi-site hospice groups?
Up to 60 FTE, yes: one site at a time, or across a small group with shared EHR. For larger multi-site work we recommend the Ongoing Compliance Partner engagement, which runs per-site in parallel with central governance.
What happens after the 90-day support window closes?
Most clients transition to the Ongoing Compliance Partner retainer for continued chart audit cadence and quarterly re-certification. Some run the system themselves for a year and re-engage before their next full survey. Either is fine, the system is designed to be run by your team, not rented.
Can I see a sample deliverable before signing?
Yes. On the scoping call we share one redacted CTI template, one redacted QAPI program design, and the sample triage report format so you can see the work before committing.

The Readiness Program sits in the middle. Lighter and faster on one side, continuous on the other.
