Irene Soirassot-Joseph, MSN, RN, built ILS CARE after watching too many strong clinical teams get punished by documentation systems that could not defend the care they were already giving. As a CHAP Certified Consultant and former Joint Commission nurse surveyor, she has read the citations from both sides of the table. ILS CARE is how she makes sure that pattern never happens to yours.

The origin story
Why ILS CARE exists.
Irene has lived every role in the chain: bedside nurse, clinical supervisor, DON, consultant. ILS CARE is what she wishes she had been able to hire.
Most compliance failures are not clinical failures. They are documentation failures dressed up as clinical ones. Nurses do the right work and then struggle to prove it on paper. Administrators catch the fallout in an ADR letter that arrives six months later, asking for money back.
Irene spent her first decade in hospice and home health watching that pattern repeat. Strong teams, good care, and then a surveyor finding that read like the care never happened. The work was there. The proof was not.

She left operational leadership in 2019 to build what she could not find: a compliance partner that did the work the way a clinician would, discipline by discipline, chart by chart, and then translated the findings into the operational and financial language an administrator could actually act on.
The D.O.C.U.M.E.N.T framework came out of that work. So did the book. So did every templated CTI, QAPI program, and corrective action plan ILS CARE ships today. Every engagement is still delivered by Irene personally, not because it is scalable, but because it is the only way to make sure the output matches the standard.
